Presbyterian Healthcare Services is seeking a compassionate and evidence-driven Physical Therapist to join our Healthcare & Medical Services team in our Albuquerque, NM network. As a key member of an interdisciplinary care team, you will evaluate, plan, and deliver high-quality physical therapy services to a diverse patient population across the continuum of care, including acute, outpatient, and/or rehabilitation settings based on organizational needs. You will provide detailed musculoskeletal and neuromuscular assessments, develop individualized treatment plans, and implement therapeutic interventions designed to restore function, improve mobility, reduce pain, and enhance overall quality of life. In this role, you will collaborate closely with physicians, nurses, occupational and speech therapists, case managers, and other healthcare professionals to coordinate safe, patient-centered care that aligns with Presbyterian’s commitment to quality, safety, and community health improvement. You will educate patients and their families about diagnoses, treatment plans, home exercise programs, injury prevention, and strategies for maximizing independence, while maintaining clear, timely documentation in the electronic health record in accordance with regulatory and organizational standards. The Physical Therapist will uphold the values of respect, integrity, accountability, and patient-first service by using evidence-based practice, clinical judgment, and outcome measures to guide treatment progression and discharge planning. You will participate in interdisciplinary rounds, contribute to care pathways and quality initiatives, and support smooth transitions of care across settings.
