As an Occupational Therapist at PruittHealth, you will provide compassionate, evidence-based therapy services to patients across our continuum of care, including skilled nursing, rehabilitation, home health, and hospice settings. You will evaluate each patient’s functional abilities, develop individualized treatment plans, and implement interventions designed to improve independence, safety, and overall quality of life in areas such as self-care, mobility, cognition, and daily living skills. Working closely with an interdisciplinary healthcare team, you will collaborate with physicians, nurses, physical and speech therapists, social workers, and family members to coordinate care, set measurable goals, and monitor progress. In this role, you will perform thorough occupational therapy evaluations, document findings clearly and accurately in the electronic medical record, and regularly update care plans based on patient response and clinical best practices. You will educate patients and caregivers on therapeutic activities, use of adaptive equipment, home safety strategies, and energy conservation techniques to support successful transitions across care settings and back into the community. You will also participate in discharge planning, making recommendations for continued services, equipment needs, and environmental modifications.
