Physical Therapist

PruittHealth is seeking a compassionate and patient-focused Physical Therapist to join our Healthcare & Medical Services team. As a key clinical professional within our rehabilitation program, you will provide evidence-based physical therapy interventions to help patients restore function, reduce pain, improve mobility, and maximize independence across a variety of care settings. In this role, you will perform comprehensive patient evaluations, including detailed musculoskeletal, neuromuscular, and functional assessments. You will develop and implement individualized plans of care that align with physician orders, organizational protocols, and current best practices. Using therapeutic exercise, manual therapy, gait and balance training, modalities, and patient education, you will guide patients through each phase of their rehabilitation journey. You will collaborate closely with an interdisciplinary team that may include physicians, nurses, occupational and speech therapists, social workers, and case managers to coordinate patient-centered care. You will regularly document assessments, treatment sessions, progress notes, and discharge summaries in the electronic medical record, ensuring accuracy, clarity, and regulatory compliance. As a Physical Therapist at PruittHealth, you will provide clear education and instruction to patients, families, and caregivers on home exercise programs, safety strategies, mobility techniques, and equipment use to support successful transitions across care settings and promote long-term functional gains. You will monitor patient progress, reassess functional status, and adjust treatment plans as needed to optimize outcomes. You will also contribute to quality improvement initiatives, adhere to all state and federal regulations, and follow PruittHealth policies related to documentation, infection control, safety, and ethical practice. You may assist in orienting and mentoring new team members, therapy assistants, and students, supporting a collaborative, learning-focused environment.
