Avera Health is seeking a compassionate, patient-centered Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide high-quality, evidence-based physical therapy across the continuum of care, helping patients restore function, reduce pain, and improve mobility and quality of life. You will evaluate patients using standardized assessments, develop individualized treatment plans, and deliver a full range of therapeutic interventions including therapeutic exercise, manual therapy, neuromuscular re-education, gait and balance training, and patient and family education. Working collaboratively within Avera Health’s interdisciplinary team of physicians, nurses, occupational and speech therapists, case managers, and other healthcare professionals, you will contribute to coordinated care plans that reflect the organization’s culture of compassion, hospitality, and stewardship. You will document all care promptly and accurately in the electronic medical record, track progress toward functional goals, and adjust treatment plans based on patient response and best-practice guidelines. The Physical Therapist will provide care to a diverse patient population that may include orthopedic, neurologic, cardiopulmonary, post-surgical, and geriatric patients in both inpatient and outpatient settings, depending on organizational needs. Responsibilities include conducting initial and follow-up evaluations, setting measurable functional goals, educating patients and caregivers on home exercise programs, fall prevention, safe mobility, and use of assistive devices, and actively participating in discharge planning to support safe transitions of care.
