LifeBridge Health is seeking a compassionate and organized Social Worker Case Manager to join our Healthcare & Medical Services team within our regional healthcare network in Maryland. In this role, you will provide patient-centered social work and case management services across the continuum of care, helping patients and families navigate complex medical, social, and community systems. You will conduct psychosocial assessments, identify barriers to care, and develop individualized care plans that address medical, behavioral, social, and environmental needs. Working closely with physicians, nurses, therapists, and community partners, you will coordinate services such as discharge planning, home health, rehabilitation, behavioral health, financial and housing resources, and other community supports. You will advocate for patients and families, provide counseling and education on coping with illness, adjustment, and resources, and help ensure safe, timely transitions between care settings. The Social Worker Case Manager will utilize evidence-based practice, motivational interviewing, and crisis intervention skills to support patient engagement, self-management, and overall well-being. You will maintain accurate, timely documentation in the electronic health record and case management systems while tracking outcomes and resource utilization.
