Avera Health, a regional health system based in Sioux Falls, SD, is seeking a compassionate and skilled Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will provide patient-centered occupational therapy services to individuals across the lifespan, helping them regain, develop, and maintain the daily living and work skills needed for maximum independence and quality of life. Working collaboratively with physicians, nurses, rehabilitation professionals, and other members of the care team, you will perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions in alignment with Avera Health’s mission of compassion, hospitality, and stewardship. You will assess functional abilities, including activities of daily living, motor skills, cognition, and sensory processing, and design therapeutic programs that may include therapeutic exercise, functional task training, adaptive equipment recommendations, and environmental modifications. The Occupational Therapist will educate patients, families, and caregivers on home exercise programs, safety strategies, and adaptive techniques to support safe discharge and long-term functional success. You will document assessments, treatment sessions, goals, and progress accurately and timely within the electronic medical record, ensuring compliance with regulatory standards and payer requirements. In this position, you may provide services in a variety of settings, such as acute care, inpatient rehabilitation, outpatient clinics, or transitional and long-term care facilities within the Avera Health network. As part of a collaborative and supportive team, you will participate in interdisciplinary care conferences, contribute to discharge planning, and communicate regularly with referring providers regarding patient status and progress. You will also engage in ongoing professional development, quality improvement initiatives, and evidence-based practice activities to continuously enhance care quality and patient outcomes.
