Primary Care Physician-OSH

Oak Street Health is seeking a compassionate, mission-driven Primary Care Physician (Primary Care Physician-OSH) to join our Healthcare & Medical Services team. In this role, you will provide comprehensive, longitudinal primary care to an adult Medicare population, with a strong focus on prevention, chronic disease management, and closing care gaps in underserved communities. You will work in a multidisciplinary, team-based environment that includes nurses, medical assistants, care managers, social workers, and behavioral health professionals, all aligned around delivering personalized, high-quality care. As a Primary Care Physician at Oak Street Health, you will maintain a panel of adult patients, perform thorough history and physical examinations, develop evidence-based care plans, and manage acute and chronic conditions such as diabetes, hypertension, COPD, heart disease, and other common primary care issues. You will conduct in-person and, when appropriate, virtual visits, coordinate specialty referrals, interpret diagnostic tests and laboratory studies, and ensure appropriate follow-up. You will emphasize preventive care, including immunizations, cancer screenings, and lifestyle counseling, while striving to improve health outcomes and reduce avoidable hospitalizations and emergency department visits. You will collaborate closely with your care team in daily huddles and regular case conferences to review high-risk patients, address social determinants of health, and design personalized interventions. The Primary Care Physician-OSH role is a full-time opportunity based primarily in a field/on-site clinic environment, with some flexibility for hybrid care models as needed. You will typically work a consistent weekly schedule with limited after-hours responsibilities and shared call coverage, depending on clinic needs.
