Primary Care Physician-OSH

Oak Street Health is seeking a compassionate, mission-driven Primary Care Physician-OSH to join our Healthcare & Medical Services team. As a Primary Care Physician at Oak Street Health, you will focus on delivering high-quality, value-based care to adult patients on Medicare in underserved communities. You will work within a multidisciplinary, team-based environment that emphasizes prevention, continuity, and relationship-based care rather than volume. In this role, you will manage a dedicated panel of adult patients, providing comprehensive primary care including chronic disease management, preventive care, acute care, and care coordination across settings. You will conduct thorough patient assessments, develop personalized care plans, perform routine follow-ups, and leverage evidence-based guidelines to improve health outcomes. You will work closely with nurses, care managers, behavioral health professionals, and other clinicians to ensure that each patient receives holistic, patient-centered care. You will use electronic health records and population health tools to identify care gaps, track outcomes, and proactively engage patients in their healthcare. You will coordinate with specialists, hospitals, and community resources to reduce avoidable hospitalizations, improve access to needed services, and support patients and caregivers through complex care journeys. Education and communication are central to this role; you will counsel patients on lifestyle, medication adherence, and disease self-management while fostering trust and long-term relationships.
