PruittHealth is seeking a compassionate and motivated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide high quality, evidence-based rehabilitative care to adult and geriatric patients in a skilled nursing and post-acute environment. Working as part of an interdisciplinary care team, you will evaluate patients’ functional status, develop individualized treatment plans, and deliver therapeutic interventions that promote mobility, strength, pain reduction, and improved quality of life. The Physical Therapist will perform comprehensive assessments, including range of motion, strength, balance, gait, and functional mobility evaluations, and will use clinical judgment to set measurable, patient-centered goals. You will design and implement treatment programs using therapeutic exercise, manual therapy, neuromuscular re-education, gait training, and modalities as appropriate, while ensuring safety and adherence to physician orders and regulatory guidelines. Ongoing reassessment of progress, timely modification of care plans, and clear communication of outcomes will be essential. You will collaborate closely with physicians, nurses, occupational and speech therapists, social workers, and other members of the care team to coordinate and optimize patient care. This includes participating in care conferences, contributing to discharge planning, and providing education to patients, families, and caregivers on home exercises, fall prevention, and mobility strategies to support safe transitions of care. Accurate and timely documentation of evaluations, daily treatments, progress notes, and discharge summaries in the electronic health record is required.
