Primary Care Physician-OSH

Oak Street Health is seeking a compassionate and mission-driven Primary Care Physician-OSH to join our Healthcare & Medical Services team. In this role, you will provide comprehensive, longitudinal primary care to an adult Medicare population, with a strong focus on underserved communities. You will deliver patient-centered, preventative care that emphasizes early intervention, chronic disease management, and coordinated services to improve health outcomes and reduce avoidable hospitalizations. As a Primary Care Physician at Oak Street Health, you will manage a panel of adult patients, conducting thorough history and physical examinations, developing individualized care plans, and following evidence-based clinical guidelines. You will diagnose and treat acute and chronic conditions, order and interpret diagnostic tests, manage medications, and coordinate care with specialists, hospitals, and community resources. You will leverage longer appointment times and smaller panel sizes to build strong, trusting relationships with patients and their families. You will work within a highly collaborative, team-based model that includes nurse practitioners, physician assistants, nurses, care managers, social workers, and other allied health professionals. Together, you will address medical, behavioral, and social determinants of health, including transportation, medication adherence, and access to community support services. You will participate in daily huddles, interdisciplinary case conferences, and quality improvement initiatives to optimize care delivery and patient experience. Preventative care and population health management are central to this position. You will proactively close care gaps, perform wellness and annual Medicare visits, provide age-appropriate screenings and immunizations, and use data-driven tools and electronic health records to monitor key quality metrics. You will educate patients on lifestyle modification, disease prevention, and self-management, empowering them to take an active role in their health.
