Presbyterian Healthcare Services is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. Based in Albuquerque, NM, Presbyterian is a non-profit healthcare system committed to exceptional care quality, community health improvement, and a culture rooted in patient-care, respect, integrity, accountability, and safety. In this role, you will provide evidence-based physical therapy services to a diverse patient population across the continuum of care, including acute, outpatient, and/or rehabilitation settings depending on assignment. You will complete comprehensive examinations and evaluations, develop individualized plans of care, and deliver interventions designed to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. Working in a highly collaborative environment, you will partner closely with physicians, nurses, occupational and speech therapists, case managers, and other members of the interdisciplinary team to coordinate patient-centered care. You will educate patients and their families on treatment plans, home exercise programs, injury prevention, and strategies to promote long-term health and wellness. Clear, timely documentation of assessments, goals, progress, and outcomes in the electronic medical record is an essential part of this position. You will be expected to apply current clinical guidelines and best practices, utilize appropriate outcome measures, and adjust treatment plans based on patient response and changing medical status. The role includes participation in care conferences, discharge planning, and safe transition of patients to home or other levels of care. You may also assist with student mentoring, staff in-services, and quality-improvement or performance-improvement initiatives that support Presbyterian’s commitment to quality and safety.
