Oak Street Health is seeking a compassionate, mission-driven Primary Care Physician – Bilingual to join our Healthcare & Medical Services team. As a core member of our primary care practice, you will provide comprehensive, patient-centered medical care to adults on Medicare, with a special focus on individuals living in underserved communities. You will deliver high-quality, evidence-based care using a preventative, population-health approach designed to improve outcomes and reduce avoidable hospitalizations. In this role, you will manage a panel of adult and senior patients, performing routine and complex primary care, including wellness visits, chronic disease management, acute care, and care coordination across settings. You will conduct thorough assessments, develop personalized care plans, order and interpret diagnostic tests, prescribe medications, and coordinate with specialists and external providers as needed. Because many of our patients are more comfortable communicating in a language other than English, you will provide bilingual care, using your language skills to build trust, enhance understanding, and support shared decision-making. You will work as part of a highly collaborative, team-based care model that includes nurses, medical assistants, care managers, social workers, and other allied health professionals. Together, you will develop and execute integrated care plans that address patients’ medical, behavioral, and social needs. You will actively participate in daily huddles, case reviews, and quality improvement initiatives, contributing your clinical expertise and cultural competence to help the team deliver exceptional patient experiences. Oak Street Health emphasizes preventative healthcare and population health management. You will be expected to use data, clinical guidelines, and population health tools to proactively identify care gaps, close preventive screening measures, optimize chronic disease control, and reduce hospital and emergency department utilization. You will leverage electronic health records and clinical decision support tools to document visits, track outcomes, and coordinate care efficiently and accurately. As a Primary Care Physician – Bilingual, you will also play a key role in community health by building long-term, trusting relationships with patients and their families. You will educate patients about their conditions and treatment options, promote healthy lifestyle changes, and connect them with appropriate community resources. Your ability to communicate clearly and compassionately in more than one language will be essential in reducing barriers to care and improving health literacy.
