In this role, you will lead a multidisciplinary care team that may include nurse practitioners, medical assistants, care managers, social workers, and other clinical staff. Working in a collaborative, patient-centered environment, you will develop and implement individualized care plans, perform thorough assessments, interpret diagnostic tests, and use evidence-based guidelines to manage a wide range of medical conditions. You will focus on proactive, preventative care that reduces avoidable hospitalizations, improves health outcomes, and enhances quality of life for our patients.
You will utilize electronic health records and population health tools to track outcomes, identify care gaps, and manage panels of patients. Regular responsibilities include conducting in-person and, when appropriate, virtual visits; ordering and reviewing labs, imaging, and referrals; providing clear patient education; and coordinating with specialists, hospitals, and community resources. You will be expected to participate in quality improvement initiatives, contribute to clinical protocols, and collaborate with colleagues to continually enhance care delivery.

This is a full-time, primarily on-site clinical role in a modern primary care center, with potential for limited hybrid or telehealth work depending on organizational needs and patient demand.
