In this role, you will conduct thorough patient assessments, perform routine and problem-focused visits, manage complex chronic conditions, and coordinate care across a multidisciplinary team that may include nurses, medical assistants, care managers, behavioral health specialists, and social workers. You will use your Spanish fluency to communicate directly and effectively with Spanish-speaking patients and families, ensuring clear understanding of diagnoses, treatment plans, medications, and preventive care recommendations.
You will utilize evidence-based guidelines, clinical decision support tools, and population health data to proactively manage your panel, close care gaps, and tailor care plans to each patient’s medical, social, and behavioral needs. Responsibilities include ordering and interpreting diagnostic tests, prescribing medications, delivering vaccinations and preventive screenings, and supporting transitions of care after hospital or skilled nursing facility stays. You will document encounters accurately and timely in the electronic health record and collaborate closely with care team members to ensure continuity and quality of care.
