In this role, you will conduct thorough patient assessments, perform routine and complex primary care visits, and develop individualized care plans that reflect each patient’s medical, social, and behavioral health needs. You will collaborate closely with nurses, care managers, behavioral health specialists, and other team members to deliver integrated, value-based care. Responsibilities include ordering and interpreting diagnostic tests, prescribing medications, managing chronic conditions such as diabetes, hypertension, COPD, and heart disease, and providing acute care for common primary care complaints.
You will leverage evidence-based guidelines and clinical decision support tools to guide care, while maintaining meticulous and timely documentation in the electronic health record. Preventative health will be central to your work, including closing care gaps, ensuring adherence to screening and immunization guidelines, and educating patients on lifestyle changes to support long-term wellness. You will also participate in quality improvement initiatives aimed at enhancing patient experience, clinical quality, and population health outcomes.
