Sanford Health is seeking a compassionate and detail‑oriented RN, Minimum Data Set (MDS) nurse to support high‑quality resident care across our senior living and post‑acute facilities within our Healthcare & Medical Services department. As an MDS RN, you will use your clinical expertise and knowledge of regulatory guidelines to accurately assess residents, complete comprehensive MDS assessments, and support individualized care planning that reflects each resident’s needs and goals. Working closely with interdisciplinary team members—including physicians, therapists, social workers, dietitians, and nursing staff—you will coordinate assessment schedules, gather and validate clinical data, and ensure timely completion and submission of MDS assessments in accordance with federal, state, and facility policies. You will review medical records, interview residents and families when appropriate, and translate clinical findings into accurate MDS documentation that supports quality outcomes, appropriate reimbursement, and regulatory compliance. In this role, you will monitor changes in resident condition and initiate appropriate assessment updates, collaborating with the care team to revise care plans and support safe, person‑centered care. You will participate in quality improvement initiatives, audits, and performance reviews related to MDS accuracy, case mix, and quality measures, using data to help drive improvements in clinical practice, safety, and resident satisfaction. Your work will include educating and supporting nursing and interdisciplinary staff on MDS processes, care documentation standards, and regulatory requirements, serving as a resource for questions related to RAI (Resident Assessment Instrument), PDPM, and related guidelines. You will also partner with leadership on survey readiness, helping to ensure that documentation and assessments reflect current standards and best practices.
