Sanford Health is seeking a dedicated RN, Minimum Data Set (MDS) nurse to support our senior living and post‑acute care services within our Healthcare & Medical Services department. In this role, you will be responsible for coordinating, completing, and submitting comprehensive MDS assessments that accurately reflect each resident’s clinical status, functional abilities, and care needs. You will collaborate closely with interdisciplinary team members, including physicians, therapists, social workers, and nursing staff, to ensure that assessment data is current, compliant with federal and state regulations, and aligned with individualized care plans. Using your clinical judgment and knowledge of MDS and Resident Assessment Instrument (RAI) guidelines, you will review medical records, conduct resident interviews and observations, and validate documentation to support accurate coding for quality measures and reimbursement. You will monitor assessment schedules, track due dates, and ensure timely completion and electronic submission of MDS assessments, including admission, quarterly, annual, significant change, and discharge assessments. The RN, MDS will educate and support nursing staff and other team members on proper documentation practices, changes in MDS regulations, and quality improvement opportunities. You will help analyze quality indicator reports, identify trends, and partner with leaders to implement performance improvement initiatives that enhance resident outcomes and satisfaction.
