We are seeking a dedicated *Physical Therapist (PT)* to join our home health team serving Orleans and northern Essex counties of Vermont. In this role, you will provide personalized, evidence-based care to patients in their homes, helping improve mobility, reduce pain, and enhance overall quality of life.
This position offers flexibility, autonomy, and the opportunity to work with a diverse patient population, including pediatric and geriatric patients.
*Responsibilities*
* Evaluate patients and develop individualized treatment plans
* Provide hands-on therapy including orthopedic rehab and trigger point therapy
* Deliver care to patients across various age groups
* Coordinate with healthcare teams on care plans and discharge planning
* Document patient progress using EMR systems
* Educate patients and families on exercises, recovery plans, and injury prevention
* Adjust treatment plans based on patient progress and needs
*Requirements*
* Active *Physical Therapist license in Vermont* (required)
* Experience in home health, outpatient, or inpatient settings
* Strong knowledge of anatomy, physiology, and rehabilitation techniques
* Experience with EMR documentation systems
* Excellent communication and patient education skills
* Ability to work independently and manage time effectively
* Reliable transportation to travel within service area
*Preferred Qualifications*
* Home health experience
* Experience treating pediatric and geriatric patients
* Familiarity with discharge planning and multidisciplinary care coordination
Pay: $33.58 - $54.82 per hour
