Description:

The Outreach Specialist connects individuals in the community with substance use and mental health treatment services through direct field outreach and relationship-building. This role provides education, peer support, advocacy, and assistance in navigating treatment and community resources. The specialist also develops referral partnerships and maintains accurate documentation to support access to care and program effectiveness.

Requirements:

Outreach & Relationship Building

  • Develop and maintain relationships with referral sources, including hospitals, healthcare providers, and community organizations.
  • Conduct in-person and virtual outreach visits to promote services and maintain engagement.
  • Serve as a point of contact for referral partners and respond to inquiries in a timely manner.

Community Engagement

  • Represent the organization at community events, meetings, and networking opportunities.
  • Assist in coordinating outreach events, presentations, and educational initiatives.
  • Promote awareness of behavioral health and substance use treatment services within the community.

Admissions Support

  • Collaborate with the Admissions team to support the referral and intake process.
  • Facilitate communication between referral sources and internal teams to ensure a smooth transition into care.
  • Track referral activity and assist with follow-up communication as needed.

Compliance & Ethical Practices

  • Ensure all outreach activities comply with applicable federal and state regulations, including patient privacy and anti-kickback laws.
  • Maintain confidentiality of patient and referral source information.
  • Promote ethical practices that prioritize appropriate level of care and patient well-being.

Reporting & Documentation

  • Maintain accurate records of outreach activities, contacts, and referral data.
  • Assist in tracking outreach performance metrics and reporting outcomes to leadership.
  • Utilize CRM systems and internal tools to document and manage outreach efforts.

Qualifications


Required

  • Education: High school diploma or equivalent required; Associate’s or Bachelor’s degree in Business, Marketing, Healthcare, or a related field preferred.
  • Experience: 1–3 years of experience in outreach, sales, customer service, or a related field preferred. Experience in behavioral health or healthcare is a plus.
  • Technical Skills :
    • Basic proficiency in Microsoft Office (Word, Excel, Outlook)
    • Experience with CRM systems or willingness to learn
