LPN -Office Nurse

Mohawk Valley Health System (MVHS) is seeking a compassionate, detail‑oriented LPN – Office Nurse to join our Healthcare & Medical Services department. As an integral member of a multidisciplinary care team, the LPN – Office Nurse supports physicians and advanced practice providers in a busy outpatient setting, delivering high‑quality, patient‑centered care to individuals and families in Upstate New York. In this role, you will room patients, obtain and document vital signs and medical histories, reconcile medications, and prepare patients for examinations, procedures, and treatments. You will administer medications and injections as ordered, perform basic clinical procedures such as EKGs, wound care, point‑of‑care testing, and specimen collection, and ensure accurate, timely documentation in the electronic health record in accordance with MVHS policies and regulatory requirements. The LPN – Office Nurse provides patient education on diagnoses, treatment plans, medications, and follow‑up care, reinforcing provider instructions and promoting health maintenance and disease prevention. You will coordinate patient flow, schedule follow‑up visits and diagnostic tests, assist with care coordination, and respond to patient calls and portal messages, escalating concerns appropriately. The position requires close collaboration with physicians, RNs, medical assistants, and administrative staff, and active participation in quality, safety, and performance improvement initiatives.
