Nurse Practitioner- PRN

Matrix Medical Network is seeking a compassionate and autonomous Nurse Practitioner – PRN to join our Healthcare & Medical Services team. In this role, you will deliver high-quality, patient-centered care through in-home and community-based health assessments, helping to improve outcomes and reduce healthcare costs for the individuals and populations we serve. Working with a leading healthcare services provider that partners with insurance plans and government agencies, you will use your clinical expertise to complete comprehensive health risk assessments, support chronic disease management, and identify gaps in care. As a PRN Nurse Practitioner with Matrix Medical Network, you will conduct face-to-face visits in patients’ homes or community settings, performing thorough histories and physical exams, reviewing medications, and documenting findings using electronic health records. You will evaluate risk factors, screen for undiagnosed conditions, provide patient education on disease prevention and self-management, and make appropriate referrals to primary care and specialty providers when necessary. Your work will help guide care plans and inform quality and population health initiatives. You will collaborate with a supportive interdisciplinary team while working with a high level of independence in the field.
