Physician Assistant

Matrix Medical Network is seeking a compassionate and clinically skilled Physician Assistant to join our Healthcare & Medical Services team, delivering high-quality care primarily through in-home and community-based health assessments. As a key clinical member of Matrix Medical Network, you will leverage your advanced training and judgment to perform comprehensive health evaluations, manage chronic conditions, and support preventive care for diverse patient populations, including members of insurance plans and government-sponsored programs. Working in a field-based role, you will travel to patients’ homes and designated community locations to conduct detailed histories and physical examinations, order and interpret appropriate diagnostic tests, and develop evidence-based care plans that promote improved health outcomes and reduced healthcare costs. In this position, you will partner closely with physicians, nurses, and other healthcare professionals to coordinate care, close quality gaps, and ensure appropriate follow-up, while documenting all encounters thoroughly and accurately in electronic medical records. You will provide clear patient education, counseling individuals and families on disease management, medication adherence, lifestyle modification, and preventive services, always maintaining a patient-centered and culturally sensitive approach. Your work will directly support Matrix Medical Network’s mission to transform healthcare delivery by combining clinical expertise with innovative models of care. You will contribute to quality improvement initiatives, participate in case reviews, and help identify opportunities to enhance efficiency, accuracy, and patient satisfaction.
