Home Health Speech Language Pathologist

CenterWell Home Health is seeking a compassionate and skilled Home Health Speech Language Pathologist to provide personalized, in-home therapeutic services to adult and geriatric patients. In this role, you will travel to patients’ homes to assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders, developing individualized care plans that promote safety, independence, and improved quality of life. You will collaborate closely with an interdisciplinary team—including nursing, physical therapy, occupational therapy, and medical providers—to coordinate care, monitor progress, and adjust treatment as needed. Responsibilities include conducting comprehensive evaluations, delivering evidence-based speech, language, voice, and swallowing therapies, educating patients and caregivers on strategies and home exercise programs, and accurately documenting visits in accordance with regulatory and CenterWell standards.
