Matrix Medical Network is seeking a compassionate and detail-oriented Physician Assistant to join our Healthcare & Medical Services team, delivering high-quality care to health plan members in their homes and through virtual visits. As a key clinical member of our organization, you will perform comprehensive health assessments, manage chronic and acute conditions within your scope, and partner with patients, families, and care teams to improve outcomes and reduce avoidable healthcare utilization. You will conduct focused histories and physical examinations, review medications, order and interpret appropriate diagnostic tests, and develop evidence-based care plans that support risk reduction, early intervention, and wellness. In this role, you will work closely with physicians, nurses, care coordinators, and health plan partners, documenting accurately in electronic health records and contributing to quality and risk-adjustment initiatives. You will educate patients on disease management, preventive care, and available community resources, helping them navigate complex healthcare systems with confidence. The position may include a mix of in-home assessments within the community and remote work, leveraging telehealth and digital tools to extend access to care.
