Physician Assistant

Matrix Medical Network is seeking a compassionate and clinically skilled Physician Assistant to join our Healthcare & Medical Services team. In this role, you will provide high-quality, patient-centered care primarily through in-home health assessments and follow-up visits for members of health plans and government-sponsored programs. As part of a multidisciplinary care team, you will leverage your clinical expertise to evaluate health status, identify gaps in care, support chronic disease management, and contribute to overall care plans designed to improve outcomes and reduce avoidable healthcare costs. Working independently in the field, you will conduct comprehensive medical histories and physical examinations, review and reconcile medications, order and interpret appropriate diagnostic tests within your scope, and document findings accurately in electronic health records. You will educate patients and caregivers on condition management, preventive care, lifestyle changes, and appropriate use of healthcare resources, while collaborating closely with physicians, nurses, care coordinators, and quality teams to ensure continuity and quality of care. You will help translate quality metrics and clinical guidelines into actionable plans that enhance patient well-being and support value-based healthcare initiatives. This position is ideal for a Physician Assistant who is adaptable, enjoys working autonomously in community and home settings, and is motivated by direct impact on patient lives.
