Nurse Practitioner- PRN

Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner – PRN to join our Healthcare & Medical Services team. In this role, you will provide high-quality clinical care to health plan members through in-home assessments, field-based visits, and other care settings as needed. You will use your advanced clinical training and judgment to evaluate patient health, manage acute and chronic conditions, and support preventive care initiatives that improve outcomes and reduce overall healthcare costs. As a Nurse Practitioner with Matrix Medical Network, you will perform comprehensive health assessments, including detailed histories and physical examinations, for diverse adult and geriatric populations. You will order and interpret appropriate diagnostic tests, review medications, identify risk factors, and develop evidence-based care plans in collaboration with primary care providers, specialists, and care management teams. Your findings will support health plans and government agencies in understanding member needs, closing care gaps, and enhancing quality and compliance metrics. This PRN position offers flexible, as-needed scheduling, making it ideal for an experienced clinician who values autonomy and variety in their practice. You will travel to patient homes and community locations within your assigned area, using strong interpersonal and communication skills to build rapport, educate patients and families, and encourage engagement in recommended care. Accurate and timely documentation in electronic health records (EHR) will be essential to ensure clear communication and continuity of care.
