Life Care Centers is seeking a compassionate, patient-focused Certified Occupational Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will work under the supervision of a licensed Occupational Therapist to provide hands-on rehabilitative care for primarily senior patients in a post-acute, skilled nursing, and long-term care environment. You will carry out individualized treatment plans designed to improve functional independence, safety, and quality of life, with a strong focus on activities of daily living such as dressing, grooming, bathing, and functional mobility. As a Certified Occupational Therapist Assistant with Life Care Centers, you will assist with patient evaluations by gathering information, observing functional abilities, and reporting findings to the supervising therapist. You will implement therapeutic exercises and activities, guide patients in the safe use of adaptive equipment, and reinforce techniques that support strength, coordination, balance, and cognitive engagement. You will carefully monitor patient responses, document progress in the electronic medical record, and communicate changes in condition or performance to the therapy and nursing teams. You will collaborate daily with Occupational Therapists, Physical Therapists, Speech-Language Pathologists, nurses, social workers, and other members of the interdisciplinary team to support coordinated, patient-first care. Your work will include organizing and managing a treatment schedule, preparing therapy spaces and equipment, maintaining infection control and safety standards, and educating patients and families on home exercise programs and strategies for maintaining independence after discharge.
