Matrix Medical Network is seeking a compassionate and driven Physician Assistant to join our Healthcare & Medical Services team, providing high-quality clinical care and in-home health assessments to diverse patient populations. As a key member of our care delivery model, you will perform comprehensive health evaluations, manage acute and chronic conditions within your scope of practice, and partner with patients, families, and multidisciplinary teams to improve health outcomes and reduce avoidable healthcare utilization. In this role, you will conduct detailed histories and physical examinations, order and interpret appropriate diagnostic tests, and develop evidence-based care plans tailored to each patient’s needs. You will assess functional status, medication adherence, and social determinants of health during in-home visits, documenting findings accurately within our electronic health record system. You will provide education and counseling to patients and caregivers on disease management, lifestyle modification, and preventive care, helping them better understand and manage their conditions. Working closely with collaborating physicians, nurses, and care coordinators, you will contribute to quality initiatives and risk-adjustment programs that support health plans and government agencies. You will be expected to practice autonomously within established protocols, escalate complex or unstable cases appropriately, and communicate clearly and professionally with referring providers and clinical partners. Your clinical expertise and sound judgment will be essential in identifying early signs of deterioration and ensuring timely interventions. This position is ideal for a Physician Assistant who thrives in a field-based environment and values innovation, adaptability, and collaboration. You will travel to patients’ homes and community settings, applying strong organizational and time-management skills to manage daily schedules while maintaining excellent patient relations and safety standards.
