Nurse Practitioner- PRN

Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner – PRN to join our Healthcare & Medical Services team. In this role, you will deliver high-quality clinical care through in-home health assessments and related medical services for health plan members and government program participants. Working independently in the field, you will conduct comprehensive health evaluations, including detailed medical histories, physical examinations, and focused assessments tailored to each patient’s needs. You will identify and document chronic and acute conditions, review medications, order or recommend appropriate diagnostic tests within scope, and provide evidence-based recommendations to support optimal care and care coordination. As a Nurse Practitioner with Matrix Medical Network, you will play a key role in improving patient outcomes and helping healthcare partners better understand and manage population health risks. You will accurately document all findings in electronic medical records, ensuring compliance with regulatory, quality, and coding standards. You will collaborate with internal clinical teams, quality assurance staff, and external healthcare providers to communicate significant findings, close care gaps, and support follow-up care where appropriate. Patient education is central to this position; you will counsel individuals and caregivers on disease management, preventive care, medications, and lifestyle modifications to promote long-term wellness and reduce avoidable healthcare utilization. This PRN position offers flexible scheduling, ideal for an experienced Nurse Practitioner who values autonomy, field-based work, and direct patient interaction. You will travel within the assigned region to conduct visits in members’ homes or community settings, requiring strong organizational skills, professionalism, and comfort working independently.
