Über

As a Home Health Speech Language Pathologist with CenterWell Home Health, you will provide one-on-one, in-home speech therapy services to adult and geriatric patients, helping them regain and maximize communication and swallowing abilities in the comfort of their own homes. CenterWell Home Health provides personalized, at-home healthcare services focused on enhancing patient autonomy and quality of life through skilled nursing and therapeutic treatments, and you will be an important part of this interdisciplinary care team. In this role, you will conduct comprehensive speech, language, cognitive-communication, voice, and swallowing evaluations, develop individualized treatment plans, and deliver evidence-based interventions that promote safety, independence, and functional outcomes. You will collaborate closely with nurses, physical and occupational therapists, physicians, patients, and family caregivers to ensure coordinated, patient-centered care, while educating patients and caregivers on therapy techniques, compensatory strategies, and home exercise programs. You will be responsible for managing a home-health caseload, traveling to patient residences, assessing home environments, and adapting therapy to real-life activities and patient goals. You will document assessments, treatment sessions, progress notes, and care plans accurately and timely within the electronic medical record, complying with all regulatory, payer, and CenterWell documentation standards. Ongoing reassessment, outcome tracking, and communication with the broader care team will be central to your contribution to patient safety and quality outcomes.
