Matrix Medical Network is seeking a compassionate and detail-oriented Physician Assistant to join our Healthcare & Medical Services team, delivering high-quality care directly to patients where they live. As a key clinical member of a dynamic, innovative organization focused on transforming healthcare delivery, you will conduct comprehensive in-home and community-based health assessments, manage ongoing clinical needs, and partner with patients, families, and care teams to improve outcomes and reduce avoidable healthcare utilization. In this role, you will perform thorough health risk assessments and focused physical exams, obtain detailed medical histories, and evaluate chronic and acute conditions for diverse adult and senior populations. You will order and interpret appropriate diagnostic tests within scope, formulate evidence-based care plans, and collaborate closely with supervising physicians, nurses, care managers, and other clinicians to ensure safe, effective, and patient-centered care. You will provide medication reconciliation, assess adherence and potential interactions, and offer education and counseling to support disease management, preventive care, and lifestyle changes. Working primarily in the field, you will travel to patient homes and community settings, using your clinical judgment to identify gaps in care, safety concerns, and social determinants of health that may impact well-being. You will document all encounters accurately and promptly in our Electronic Health Record system, supporting quality assurance, regulatory compliance, and continuity of care. You will communicate findings and recommendations to referring providers and health plan partners, contributing to population health initiatives and risk adjustment accuracy.
