Physician Assistant

Matrix Medical Network is seeking a compassionate and clinically skilled Physician Assistant to join our Healthcare & Medical Services team, delivering high-quality care primarily through in-home and community-based health assessments. As a key member of our multidisciplinary clinical staff, you will leverage your advanced clinical training to perform comprehensive health evaluations, manage chronic and acute conditions within your scope of practice, and contribute to quality and care management programs that support health plans and government agencies. In this role, you will travel to members’ homes and designated community locations to conduct detailed histories and physical examinations, review medical records, and document findings using electronic health records. You will order and interpret appropriate diagnostic tests as permitted by state regulations, identify gaps in care, collaborate with supervising and consulting physicians, and develop evidence-based recommendations that promote improved outcomes and reduced healthcare costs. You will provide education and counseling to patients and caregivers on disease management, medications, preventive care, and lifestyle modifications, ensuring each encounter is patient-centered, respectful, and culturally sensitive. Working closely with Matrix Medical Network’s interdisciplinary team, you will participate in care coordination, communicate clinically relevant information to primary care providers and health plan partners, and support quality assurance initiatives by adhering to standardized protocols and documentation standards.
