Working directly with patients in their homes and community settings, you will identify gaps in care, address medication adherence and safety, reinforce preventative care and wellness strategies, and provide clear, empathetic education to patients and their families. You will collaborate closely with primary care providers, specialists, care managers, and payers to ensure continuity and quality of care, documenting all encounters accurately and efficiently within electronic health records while adhering to HIPAA and all regulatory, quality, and compliance standards.
As a key clinical member of Matrix Medical Network, you will exercise a high degree of autonomy in your day-to-day practice while being supported by a multidisciplinary team of clinicians, quality specialists, and operational staff. You will contribute to quality assurance initiatives, performance improvement projects, and innovative care delivery models designed to transform how healthcare reaches patients where they live. The position is well-suited for practitioners who are adaptable, organized, comfortable traveling within a defined geographic area, and motivated by measurable improvements in patient health and experience.

The ideal candidate is a licensed Nurse Practitioner or Physician Assistant with experience in primary care, internal medicine, geriatrics, or a related field, who is confident practicing in the field with a diverse patient population. You should be comfortable making independent clinical judgments within your scope of practice, managing complex medical and psychosocial needs, and communicating effectively with patients, caregivers, and healthcare partners.
