Job Summary:

As an Outside Appointment Scheduler, you’ll play an important role in helping seniors access the care they need. You’ll coordinate medical appointments outside of our centers, working closely with Participants, caregivers, care teams, and external providers to ensure appointments are scheduled accurately and on time.

This role is ideal for someone who enjoys independent work, problem-solving, and clear communication. You’ll be trusted to manage details, follow established processes, and support a smooth care experience for our Participants.

Hourly Wage:

$17.00 /hr. to start – $18.00 /hr. after 90 days

Schedule:

First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Duties/Responsibilities:

  • Schedule, confirm, reschedule, and track outside medical appointments, including specialists, diagnostics, imaging, labs, and other services.
  • Communicate appointment details clearly to Participants, caregivers, and powers of attorney.
  • Serve as a primary point of contact with outside providers to coordinate scheduling needs and follow-up care.
  • Complete referrals, authorizations, and related documentation accurately and on time.
  • Enter appointment information and encounters into the electronic medical record following PACE guidelines.
  • Monitor appointments to reduce delays, missed visits, or gaps in care.
  • Respond to calls and questions in a professional, courteous, and timely manner.
  • Participate in team meetings and quality improvement activities.
  • Follow established workflows, policies, and regulatory requirements while adapting to changing needs.

Education and Experience:

  • High School Diploma or GED required.
  • Experience in healthcare scheduling, care coordination, or a medical office is preferred.
  • Experience working with elderly or complex populations is a plus.
  • Comprehensive onboarding and training provided.

Physical Requirements:

  • Ability to move intermittently throughout the workday.
  • Ability to lift, push, pull, or move up to 25 pounds.
  • Ability to assist with participant evacuation if needed.
  • May encounter environmental challenges in Participant homes; safety protocols must be followed.
