*Job Summary*
We are seeking a dedicated and skilled Occupational Therapist to join our dynamic healthcare team. The ideal candidate will possess a strong foundation in patient-centered care, with expertise in assessing and improving individuals' functional abilities across diverse settings. As an Occupational Therapist at DCH, you will play a vital role in helping patients regain independence through tailored therapy plans, working with a broad spectrum of populations including pediatrics, geriatrics, and individuals with disabilities.
*Duties*
-Performs evaluations and treatments to child, adolescent, adult, and geriatric patients in a variety of settings and completes supporting documentation according to hospital policy (based on assignment).
Appropriately establishes and reviews:
* Plan of care
* Goals and priorities with team (including patient/family)
* Treatment in response to changing patient needs
* Educational needs of patient/caregivers
* Discharge plan/follow-up care needs
-Complies with department, hospital, and regulatory agency guidelines with respect to ethics, legal issues and supervisory relationships. Demonstrates professional accountability per the Occupational Therapy Code of Ethics and Standards of Practice.
-Demonstrate appropriate and effective written, verbal, and non-verbal communication with team members through the continuum of care .
-Participates in clinical program development.
-Understands and complies with universal precautions, safety and risk policies and procedures.
-Consults with other members of rehabilitation team to select treatment programs consistent with needs and capabilities of individual and to coordinate comprehensive services delivery.
-Maintains positive relationships with the health care team, patient, and family.
-Assists in preparation/maintenance of daily treatment schedule.
-Provides instruction, supervision, and guidance to students, volunteers, and ancillary staff in Rehab.
-Assesses physical environment and functional abilities of patient within actual residence or work setting.
*Requirements*
* Valid licensure or certification as an Occupational Therapist in the relevant state or region.
* Ability to develop effective care plans tailored to individual patient needs across diverse populations.
* Strong case management skills with the ability to coordinate discharge planning efficiently.
* Excellent communication skills for working collaboratively within multidisciplinary teams and engaging with patients’ families.
Pay: $40.00 - $50.00 per hour
