POSITION SUMMARY:

Our Practice is seeking to fill several open Nurse Practitioner and/or Physician Assistant positions.

The ideal candidate would have some home-based healthcare experience and have 1+ year(s) experience as a midlevel provider. Ponderosa Heart House Call is an innovative fast growing Mobile Cardiology Practice. Our practice is currently the ONLY mobile Cardiology practice that offers homebound patients the full continuum of consultation, in home testing, and when needed surgical interventions. All while recognizing the patient's mobility limitations.

We are seeking a team-player, who is confident in their assessment skill set, resourceful mid-level providers able to follow evidence-based medicine guidelines whom genuinely care about patient’s well-being. Nurse Practitioners and/or Physician Assistants who value putting the patient and their needs first

In this role, you will service patients at the locations that they call “home”. This may include assisted livings, group homes, and/or private homes within a personalized territory. Although this territory may fluctuate at times the goal will be to ensure continuity of care for our patients and the communities we serve.

Together, the team offers homebound patients access to specialty care that they may have not been able to otherwise receive.

Visit types are inclusive New Patient consultation, post-hospital follow-up, symptom assessment, longitudinal care, diagnostic review and development of treatment plan. Our specialized team offers

The Advanced Practitioner will be expected to see and treat a variety of Adult Patients with a variety of potential diagnoses focusing on cardiac health.

QUALIFICATIONS:

1. Licensed, Board-Certified Unrestricted NP or Equivalent Mid-level Provider

2. Must possess current unrestricted DEA and AZ prescribing authority

3. Possess the ability to obtain a Level One Fingerprint Clearance Card

4. Experience with DrChrono Software and some experience with Word and Excel is a plus.

5. Unrestricted Drivers License, Car Insurance, and reliable transportation a must.

RESPONSIBILITIES (include but are not limited to):

• Responsible for managing chronic & acute medical problems as well as initiating medications and other interventions to manage such problems
• Collecting subjective and objective information about the patient (patient’s history, symptoms, review of systems, and physical findings)
• Assessing the collected data to identify problems and set priorities to develop a patient specific plan of care.
• Documenting all findings and Plan of Care within the Electronic Medical Record (DrChrono )
• Diagnose & treat acute and chronic cardiac and medical conditions using assessment and theory skill sets.
• Implementing the care plan including chronic medication management and educating patients on test results and treatment plans ·
• Monitoring the patient over time during follow-up encounters to evaluate the effectiveness of the developed care plan and modify their care plan as needed to achieve optimal outcomes.
• Coordinate care with other providers and specialists, as needed
• Strive to improve health and happiness for patients and team on a daily basis
• Cultivate a climate of trust and compassion
• Comply with medical care regulations and safety standards, offering lifestyle, medical and dietary suggestions that accompany the
• management of cardiac diseases and cardiac diagnoses developing and adhering to a treatment plan,
• including writing prescriptions and dispensing medications, and provide patients with support and guidance as needed.

SKILLS AND DESIRED CHARACTERISTICS:

• One year of experience as a mobile based provider preferred as this can be a specialty in and of itself.
• Compassion, empathy and self-awareness
• Ability to multi-task, think critically, and find solutions to challenges with the assistance of the team.
• Strong leadership skills and effective communication skills
• Ability to excercise initiative and independent judgement.
• Ability to work with individuals to enlist cooperation of your team to perform/achieve a common goal.
