- Connect pregnant women, interconception women, and families of children under the age of three to services to offset risk factors that may lead to poor pregnancy outcomes and/or poor developmental outcomes
- Provide family centered, strength based interventions with sensitivity toward cultural differences
- Assess risks and link individuals to needed resources, provide information, and appropriately determine eligibility for home visiting services
- Promote all CI&R participating programs equally and without bias and ensure participant/family is referred and connected to the appropriate home visiting program
- Must administratively initiate and ensure all appropriate protocols for all newly received screens/referrals
- Adhere to timeliness standards for all documentation in the Well Family System (WFS), including coding of the Initial Intake and all other services and referrals
Must enter data and notes electronically within 3 business days of services
High school diploma or equivalent
- Experience working with or providing services to children and families
- Knowledge of normal maternal-child health, child growth/development, and parent-child relationships
- Must complete Pre-Service and In-Service Training prior to providing services
- Ability to pass a level II Background clearance and drug test.
- Valid FL drivers' license, insurance, and safe driving record.
- CBHC New Hire Orientation (NHO) completed within first two (2) months of employment.
- Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
- Ability to use a computer.
