  • Connect pregnant women, interconception women, and families of children under the age of three to services to offset risk factors that may lead to poor pregnancy outcomes and/or poor developmental outcomes
  • Provide family centered, strength based interventions with sensitivity toward cultural differences
  • Assess risks and link individuals to needed resources, provide information, and appropriately determine eligibility for home visiting services
  • Promote all CI&R participating programs equally and without bias and ensure participant/family is referred and connected to the appropriate home visiting program
  • Must administratively initiate and ensure all appropriate protocols for all newly received screens/referrals
  • Adhere to timeliness standards for all documentation in the Well Family System (WFS), including coding of the Initial Intake and all other services and referrals
  • Must enter data and notes electronically within 3 business days of services

  • High school diploma or equivalent

  • Experience working with or providing services to children and families
  • Knowledge of normal maternal-child health, child growth/development, and parent-child relationships
  • Must complete Pre-Service and In-Service Training prior to providing services
  • Ability to pass a level II Background clearance and drug test.
  • Valid FL drivers' license, insurance, and safe driving record.
  • CBHC New Hire Orientation (NHO) completed within first two (2) months of employment.
  • Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
  • Ability to use a computer.
  • Attention to detail.
  • Ability to work independently and as part of a team, in collaboration with other community partners.
  • Ability to manage stressful situations and display appropriate work demeanor and boundaries.
  • Ability to demonstrate excellent customer service.

  • Ability to safely operate a motor vehicle in all driving conditions.
  • Ability to lift up to 10 pounds.
