Über
- Coordinate and manage the MDS (Minimum Data Set) process for patients in a healthcare setting
- Ensure accurate and timely completion of all required assessments and documentation
- Collaborate with interdisciplinary teams to develop individualized care plans for patients
- Conduct regular reviews of patient records to ensure compliance with regulatory standards
- Provide case management and discharge planning services to patients transitioning between care settings
- Collaborate with home care agencies, hospitals, and nursing homes to coordinate patient care
- Utilize electronic health record systems such as Epic for medical management and documentation
Skills:
- Strong knowledge of MDS assessment process and regulations
- Familiarity with HEDIS (Healthcare Effectiveness Data and Information Set) measures and reporting
- Experience in case management and discharge planning
- Excellent communication and interpersonal skills to collaborate with interdisciplinary teams
- Ability to prioritize tasks and manage time effectively in a fast-paced environment
- Proficiency in using electronic health record systems for documentation and medical management
- RAC-CT, RAC-CTA certifications preferred
