Your Responsibilities Deliver personalized, high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities. Conduct comprehensive assessments and develop tailored care plans for older adults. Manage chronic conditions with skill and empathy for patients and families. Collaborate with cross-functional partnersincluding palliative, psych, and neuroto deliver coordinated, whole-person care. Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals. Maintain consistency in documentation, follow-through, and patient communication. What Were Looking For Active, unencumbered Physician Assistant license
in the state you will be practicing. Minimum of 1-2 years of clinical experience
(geriatrics or primary care preferred). Leadership potential or experience:
Ideally, youve managed a panel successfully and are ready to grow into leadership roles. Experience in home-based care, SNF, or ALF settings
strongly preferred. Compassionate, patient-centered approach and strong clinical judgment. Valid drivers license and reliable transportation. Compensation and Benefits Competitive Pay:
