Astrya Global, a San Diego–based medical staffing agency, is looking for a Physical Therapist for a home health role serving Santa Clara and Alameda counties. This is a direct hire opportunity with a 5-star home health agency. This role includes working closely with geriatric patients to restore mobility, reduce fall risk, and improve functional independence through skilled evaluations and individualized treatment plans. Travel to patient homes is required.

*Qualifications of a Physical Therapist *
* Current and active CA PT license
* Home health, SNF, or community-based geriatric setting experience
* Ability to independently manage a home health caseload across multiple patient acuity
* Strong clinical judgment in fall prevention, mobility, and rehab
* Experience collaborating with interdisciplinary teams (RN, OT, ST, MSW)
* Effective communication with physicians, caregivers, and partners
* Comfortable with Medicare home health documentation
* Reliable transportation and willingness to travel
*Duties/Responsibilities of a Physical Therapist: *
* Identify and meet patients’ goals and develop a Plans of Care
* Provide skilled therapy interventions aligned with physician orders and individualized plans of care
* Deliver evidence-based interventions to improve mobility, reduce fall risk, and prevent avoidable functional decline
* Develop care plans using a variety of treatment techniques
* Create fitness- and wellness-oriented programs tailored to patients’ specific needs
* Provide quality, personalized and evidence-based care, and proven interventions
* Motivate patients during treatment to help them function optimally
* Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
* Consult and practice with other health professionals
* Evaluate effects and monitor and communicate progress
* Document patient care services in Ready2Nurse's EMR System
* Contribute to accurate and timely documentation supporting Medicare compliance and quality outcomes
