Revital Rehab Services is seeking a Physical Therapist (PT) to provide high-quality rehab services at Premier at Perry Village, a skilled nursing facility in New Bloomfield, PA. This flexible, as-needed role is perfect for a dedicated clinician who wants to make a meaningful impact—on a schedule that works for them.
Your Responsibilities:
Deliver evidence-based physical therapy services within your scope of practice
Participate in evaluations and assist in developing personalized treatment plans
Implement targeted interventions that support mobility, strength, and functional independence
Document care accurately and communicate progress with the interdisciplinary team
What You’ll Bring:
Current Pennsylvania licensure as a Physical Therapist
BLS certification
