*Job Summary:*
We are seeking a dedicated and knowledgeable *PRN Home Infusion Pharmacist* to join our growing team. This role is responsible for overseeing the preparation, dispensing, and clinical monitoring of infusion medications while ensuring regulatory and safety compliance. Ideal candidates will have a passion for patient care, strong attention to detail, and the ability to work independently in a fast-paced environment.
*Responsibilities:*
* Review, interpret, and verify medication orders for home infusion therapies.
* Compound and dispense sterile preparations in compliance with USP / guidelines.
* Collaborate with nurses, physicians, and other healthcare providers on therapy plans.
* Provide clinical oversight and patient education as needed.
* Maintain accurate documentation of interventions, communication, and medication profiles.
* Participate in on-call rotations, if applicable.
*Qualifications:*
* Active Pharmacist license in Indiana
* Minimum 1–2 years of infusion or sterile compounding experience (home infusion preferred)
* Familiarity with IV therapy, pumps, and infusion devices
* Strong clinical and communication skills
* CPR Certification (preferred)
* Willingness to travel locally, if needed
