We are a home health agency located in Cridersville, OH. We are looking for a Physical Therapist to help cover our ever growing patient base. This is a part-time or PRN position.
The PT will be responsible for providing therapy services to our clients during admissions, discharges, and other needed assessments, as well as creating an individualized Plan of Care for each patient and participating in care coordination with the other disciplines associated with each patient.
Qualifications:
-Valid PT license in the State of Ohio
-Valid driver's license
-Reliable transportation is a must
