*Job Overview OT*
We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will be responsible for assessing, planning, and implementing therapeutic interventions to help patients achieve their maximum potential in daily living activities. This role involves working in our Long-Term Care, Short Term Rehab, Dementia Care and future outpatients.
Responsibilities
Conduct thorough assessments of patients' needs and abilities related to daily living activities (ADLs).
Develop and implement individualized care plans that address specific therapeutic goals.
Assist with discharge planning to ensure a smooth transition from care settings.
Collaborate with interdisciplinary teams to enhance patient care through effective case management.
Educate patients and their families on therapeutic techniques and strategies for improving function and independence.
Document patient progress and modify treatment plans as necessary based on ongoing evaluations.
Engage in early intervention strategies for children with developmental delays or disabilities.
Utilize knowledge of anatomy, physiology, medical terminology, as applicable in treatment sessions.
Skills
Proficient in functional based interventions and developing care plans tailored to individual patient needs.
Experience in discharge planning and case management within skilled nursing facilities.
Strong communication skills for effective collaboration with patients, families, and healthcare teams.

Job Type: Full-time
Pay: $50.00 - $55.00 per hour
Expected hours: 40 per week
