Join our dynamic healthcare team as an Occupational Therapist and make a meaningful difference in patients' lives by helping them regain independence and improve their quality of life. In this vital role, you will assess individual needs, develop personalized care plans, and implement therapeutic interventions across diverse settings including inpatient facilities, outpatient clinics, schools, and home health environments. Your expertise will empower individuals to achieve their fullest potential through targeted therapy and support.
*Duties*
* Conduct comprehensive evaluations to determine patients’ functional abilities and occupational performance
* Develop, implement, and adjust customized treatment plans focused on enhancing daily living activities (ADLs), motor skills, and cognitive functions
* Assist patients with activities of daily living such as dressing, grooming, feeding, and mobility to promote independence
* Collaborate with multidisciplinary teams including speech therapists, physical therapists, physicians, educators, and case managers to coordinate holistic care
* Provide discharge planning and home exercise programs to ensure continuity of care post-treatment
* Work with individuals with disabilities or developmental conditions such as autism and support early intervention initiatives
* Document patient progress accurately in medical records while adhering to confidentiality standards
*Experience*
* Skilled nursing facility experience or post-acute care background preferred for managing complex cases in inpatient or skilled nursing environments
* Hands-on experience in outpatient clinics or home health services delivering patient-centered therapy programs
* Knowledge of occupational health practices and working with individuals with disabilities across various age groups is essential
* Familiarity with case management processes, discharge planning, and developing effective care plans for diverse patient needs
* Prior involvement in speech therapy collaborations or interdisciplinary approaches to patient rehabilitation is a plus
