*Who We Are Looking For*
We are seeking a skilled and compassionate Physical Therapist (PT) to join our team. The ideal candidate will provide comprehensive assessment, treatment, and rehabilitation services to patients with various physical conditions in both clinical and/or home health environments.
Key Responsibilities:
_*Home Health Responsibilities:*_
* Perform in-home evaluations to assess patients’ physical therapy needs in their home environment
* Design and implement tailored therapy programs that accommodate patients’ home settings and daily routines
* Educate family members and caregivers on exercises and strategies to support the patient’s rehabilitation
* Monitor and document patient progress, adjusting care plans as necessary
* Advocate for necessary home modifications and equipment to enhance patient safety and independence
* Build strong rapport with patients and families to encourage engagement in the therapeutic process.
_*In-Clinic Responsibilities:*_
* Conduct thorough evaluations to assess patients’ physical abilities, limitations, and needs
* Develop and implement individualized treatment plans based on evaluation findings and patient goals
* Utilize evidence-based therapeutic techniques to improve patients’ mobility, strength, and overall functional abilities
* Educate patients and their families about treatment plans, exercises, and self-management strategies.
* Document patient progress and adjust treatment plans as necessary to optimize outcomes.
* Collaborate with a multidisciplinary team to ensure coordinated patient care
* Stay updated on best practices and advancements in physical therapy.
*Qualifications:*
* Doctor of Physical Therapy (DPT) degree from an accredited program.
* Valid Arizona licensure as a Physical Therapist
* Experience in a clinical and/or home health setting preferred
* Strong communication and interpersonal skills.
* Ability to work independently and collaboratively within a team
* Proficiency in electronic health record (EHR) systems.
*Physical Requirements:*
* Ability to lift and assist patients as needed
* Capacity to perform physical activities such as standing, walking, and bending for extended periods
