*Qualifications*
* Current Florida Physical Therapist (PT) license
* Experienced PTs and new graduates are welcome to apply
* Weekend availability is required
* Strong clinical, communication, and organizational skills
* Reliable transportation
*Responsibilities*
* Provide skilled physical therapy visits in patients’ homes
* Develop and implement individualized care plans to support patient recovery and independence
* Collaborate with the interdisciplinary care team to ensure coordinated, high-quality care
* Maintain timely and accurate documentation using our AI-enhanced EMR system
