*EDUCATION & LICENSURE: *Graduate of an accredited school and currently licensed to practice professionally in the state of Louisiana.
*EXPERIENCE: *Minimum of one year experience as a Physical Therapist; home health experience is a plus.
*QUALIFICATIONS:*
* Knowledgeable of principles and practices of physical therapy
* Ability to manage patients with different types of personalities
* Excellent observation and assessment skills
* Excellent written and verbal communication skills
* Current CPR Certification
* Current knowledge of treatment practices per discipline and drive to continually learn and grow in the profession.
*RESPONSIBILITIES:*
* Provides physical therapy care to patients in their place of residence.
* Select and adapt the appropriate plan of care, per case
* Partners with patients and their caregivers to develop a rehabilitation goal aimed at reaching the patient’s highest level of functional ability.
* Teaches the patient/family disease process and self-care needs.
* Contributes to the provision of quality care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care
* Collect data and document processes followed
* Promote patients' healthy lifestyle by improving strength, flexibility, balance, and coordination.
