Lexington Clinic is seeking an Internal Medicine, Family Medicine or Med/Ped physician to join our brand-new Enhanced Care Clinic in
Lexington, Kentucky . This is an outpatient-only opportunity.

Job Details:
- Care Coordination: Extensivists act as central points of contact, coordinating care between primary care providers, specialists, social workers, and care coordinators.
- Outpatient Management: They provide intensive outpatient treatments, including some hospital-level services like IV medications, in settings outside of the hospital.
- Transition Management: A key focus is managing the patient's transition from hospital to home, aiming to prevent setbacks and reduce the risk of readmission.
- Comprehensive Assessment: They assess a patient's complex medical needs, including psychosocial factors and social determinants of health, to develop a unified care plan
- Support System: They work with a patient's family and their support system to provide resources and ensure continuous care after discharge.
