We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including home care and facilities. As an LPN, you will work under the supervision of registered nurses and physicians to ensure that patients receive the best possible care. Your role will involve administering medications, assisting with daily living activities, and supporting patients with diverse needs.

*Duties*
* Administer medications as prescribed and monitor patients for side effects or adverse reactions.
* Provide direct patient care, including assistance with personal hygiene, mobility, and daily living activities.
* Collaborate with healthcare teams to develop and implement individualized care plans.
* Document patient progress and changes in condition using electronic health records (Epic).
* Assist in the care of patients with specific needs such as dementia care, senior care, and those with disabilities.
* Participate in sterile processing and maintain a clean and safe environment for patients.
* Support neonatal ventilator management when necessary.
* Engage with patients and families to provide education regarding health conditions and treatment plans.
* Work effectively in various settings, including Level I Trauma Centers and ophthalmology clinics.
*Qualifications*
* Current Licensed Practical Nurse (LPN) license in the state of practice.
* Experience in medication administration is required; familiarity with home care is a plus.
* Knowledge of dementia care practices and experience working with individuals with disabilities is preferred.
* Ability to work collaboratively within a healthcare team while maintaining patient confidentiality.
* Strong communication skills to effectively interact with patients, families, and healthcare professionals.
* Previous experience in sterile processing or working within a Level I Trauma Center is advantageous.
* Familiarity with electronic health record systems, particularly Epic, is beneficial.
